What does my first visit look like?

The initial evaluation is all about the whole body assessment. You came in with a specific diagnosis but there is much more to uncover. We will review your medical history, discuss your symptoms, goals, and what you would like to get out of therapy. We then perform an orthopedic examination, which may include strength, balance, coordination, pain assessment and more.

Regarding pelvic floor therapy, we may perform an internal internal pelvic floor examination based on your presenting symptoms. Internal exams ALWAYS require your consent and you may decline at any time. We will provide you with education regarding our findings, and together we plan your care accordingly.

Do you take insurance?

MasMed Rehab is an out-of-network provider EXCEPT for Medicare and Leon Medical Centers plans. Being out-of-network removes the restrictions set by insurance companies on what your care allows (freedom!). Our sessions are always one-on-one with your physical therapist, and without insurance we are treating beyond the scope of a single diagnosis. You are responsible for checking with your insurance company to determine out-of-network providers and coverage based on your plan. For out-of-network patients, we provide a Superbill option to send to your insurance for direct reimbursement.

Is out-of-network an option for me?

If we are not in-network with your insurance plan, we're happy to provide a Superbill for you to submit to your insurance for reimbursement.

Your responsibility: upfront payment at time of service & checking with your insurance company to determine out-of-network providers and coverage based on your plan

  • If your insurance denies coverage for any reason, you’ll be responsible for a cash rate of up to $175 for an evaluation and $150 for a follow-up visit.

  • If you are currently uninsured or do not have out-of-network benefits available we are still able to see you as a self pay patient.

What’s a Superbill?

A superbill is a potential reimbursement tool for out-of-network healthcare services. It is an itemized receipt that contains the necessary information for insurance claims such as procedure codes, diagnosis codes, and associated costs.

How to receive & submit a Superbill:

  • Pay Upfront: Start by paying MasMed Rehab directly for your visit(s). Save your receipt!

  • Request Your Superbill: Easily request a superbill from us, then we generate it and send it to you via email.

  • Check Your Insurance: Contact your insurance company to know your out-of-network benefits for physical therapy.

  • Submit Your Superbill: Your insurance company should provide guidance on how to submit your claim with any required documentation. Be sure to track your claim afterwards, as reimbursement will be paid directly to you.

Do I need a referral or script?

Florida is a "direct access" state which means that you can seek out Physical Therapy services without a doctor's prescription or referral. However, if treatment spans more than 30 days or 10 treatment sessions (whichever comes first), then a doctor's prescription will be required to continue. This does not pertain to our pregnancy & postpartum fitness, virtual or at-home concierge care.

Do you offer at-home or virtual physical therapy?

Yes! MasMed Rehab provides both at-home concierge physical therapy and online physical therapy options. Both services are not covered by insurance, which in turn grants us more optimal time and quality of care without the limitations of treating a specific diagnosis. What do I bring to my first appointment?

For your initial evaluation, please try to arrive 15 minutes early to check in & complete any remaining paperwork. Be sure to bring your driver’s license & form of payment. Dress comfortably for movement! We recommend athletic wear & sneakers.

Where is MasMed Rehab located?

We are located inside of a medical office building on the 5th floor. Complementary valet provided, or street parking.
